Acute kidney injury (AKI) is a frequent complication after cardiac surgery with cardiopulmonary bypass (CPB). We therefore tested the hypothesis that an abnormally high partial pressure of carbon dioxide in the blood (PCO(2)) as a measure of pulmonary oxygenation impairment and an increased release of pulmonary carbon dioxide (PCO(2)r) into the systemic circulation would be associated with the development of postoperative AKI. We determined PCO(2), PCO(2)r, and the standard variables of pulmonary and systemic haemodynamics in 101 patients undergoing CPB (n=98, acute myocardial infarction, n=3) for elective cardiac surgery. AKI was defined by a 25% increase in serum creatinine (s-creatinine) and a new need for renal replacement therapy (RRT). Partial pressure of oxygen in the arterial blood (PaO(2)) and oxygen delivery index (PaO(2)/F(I)O(2) ratio) were significantly higher in patients who developed AKI than in those who did not (P
